These Terms and Conditions ("Terms") govern your use of services provided by The Well Transportation, a non-emergency medical transportation (NEMT) company located at 5110 N. 40th St., Tampa, FL 33610 ("The Well Transportation," "we," "us," or "our").

By scheduling transportation, submitting a ride request form, or using our website at thewelltampa.com, you confirm that you have read, understood, and agree to be bound by these Terms. If you are scheduling on behalf of another person, you represent that you have authority to bind that person to these Terms.

If you do not agree to these Terms, please do not use our services.

The Well Transportation provides non-emergency medical transportation services in the greater Tampa, Florida area, including but not limited to:

• Dialysis and chemotherapy transport
• Medical appointments and follow-up visits
• Hospital admissions and discharges
• Adult daycare transportation
• Wheelchair and stretcher transport
• Pharmacy trips
• Acute care (non-emergency) transport

Our services are non-emergency in nature. We are not an emergency ambulance service. In a medical emergency, you must call 911.

We reserve the right to modify, suspend, or discontinue any service at any time without notice. We also reserve the right to refuse service to any individual who poses a safety risk to our drivers or vehicles, or who violates these Terms.

Rides must be scheduled in advance. Same-day scheduling is subject to availability. You are responsible for providing accurate pickup times, addresses, and appointment information. We are not responsible for missed appointments arising from incorrect information provided at booking.

Cancellations: If you need to cancel or reschedule a ride, please contact us as early as possible at (813) 939-7003. Late cancellations (less than 2 hours before scheduled pickup) or no-shows may result in a cancellation fee at our discretion.

Wait time: Drivers will wait up to 15 minutes at the scheduled pickup location. After this time, the ride may be marked as a no-show and the driver may be dispatched to another trip.

Our right to cancel: We reserve the right to cancel or modify rides in cases of severe weather, vehicle issues, driver unavailability, or other circumstances outside of our reasonable control. In such cases, we will make every effort to notify you promptly and arrange alternative transportation when possible.

The Well Transportation currently accepts private pay only. Rates are provided at the time of scheduling and may vary based on distance, service type, and special equipment requirements.

Payment is due as agreed at the time of booking. Failure to pay for services rendered may result in suspension of future scheduling and, where applicable, collection action.

We reserve the right to update our rates at any time. Any rate changes will be communicated to you before your ride is confirmed.

All passengers and accompanying individuals are expected to treat our drivers and staff with respect. We will not tolerate abusive, threatening, or discriminatory behavior. Violations may result in immediate termination of the trip and refusal of future service.

Passengers are responsible for their personal belongings. The Well Transportation is not liable for items lost, damaged, or left in our vehicles.

For the safety of all passengers and drivers, no smoking, vaping, or consumption of alcohol or illegal substances is permitted in our vehicles.

If a passenger requires a caregiver, escort, or personal care attendant to travel with them, this must be disclosed at the time of scheduling so we can ensure appropriate vehicle capacity.

The Well Transportation shall not be liable for indirect, incidental, special, consequential, or punitive damages arising out of or related to our services, including but not limited to missed medical appointments, medical complications, or loss of income.

Our vehicles and drivers are appropriately licensed and insured as required under Florida law. In the event of an incident during transport, our liability is limited to direct damages and is subject to our insurance coverage.

We are not responsible for delays caused by traffic, weather, road closures, or other circumstances beyond our reasonable control. We will make every effort to notify you of delays as early as possible.

Nothing in these Terms limits or excludes our liability for death or personal injury resulting from our negligence, or any liability that cannot be excluded by applicable law.

Our website is provided for informational purposes and to facilitate ride requests. You agree not to use the website to submit false, misleading, or fraudulent information. You agree not to engage in any activity that could harm, disable, or overburden our website or systems.

All content on this website, including text, images, and design, is the property of The Well Transportation and may not be reproduced without our written permission.

These Terms are governed by and construed in accordance with the laws of the State of Florida, without regard to its conflict of law provisions. Any disputes arising under these Terms shall be subject to the exclusive jurisdiction of the courts located in Hillsborough County, Florida.

We reserve the right to update these Terms at any time. When we do, we will update the effective date at the top of this page. Your continued use of our services after any changes constitutes acceptance of the updated Terms.

We encourage you to review these Terms periodically, particularly before scheduling transportation.
